Now I'm adding components in a bit at a time until I figure out which one is triggering the issue because it doesn't exist on its own.

Enchantments:

+4 Enhancement Bonus: Weapons and Shields: +4 Enhancement Bonus Attack and Damage rolls. Armor and Shields: +4 enhancement bonus to Armor Class.
Ghostbane III: On Hit: 3d10 Bane Damage vs. Undead. (Bane damage cannot be resisted.)
Passive: Attacks from this weapon bypass the miss chance of incorporeal creatures, such as ghosts, shadows, and wraiths.
Evil Outsider Bane 3: This weapon deals 3d10 Bane damage against Evil Outsider.
Vorpal: Passive: +0.5[W] damage dice.
On Vorpal Hit: If your target has fewer than 1000 Hit Points, they are Instantly Slain. If your target has above 1000 Hit Points, they take 100 damage.
Crushing 7: This weapon deals 7d6 Force Damage on Critical Hit.

Material: This item is made out of: Force
Hardness: 17 Durability: 170

The residual power left behind as thousands of Spectral Dragon Scales crumbled to dust formed this ethereal weapon.

After the above test, it becomes clear that the only difference is Spectral Great Axe (ML 16) which is helpful information, now I have a better idea of what to look at.
